MSOA responds to proposed UHC rules for spine surgeons
Beginning March 1, 2010, orthopaedic physicians performing inpatient spinal surgeries on UnitedHealthcare members may receive requests to forward medical records. In selected cases, orthopaedic physicians may be required to participate in peer-to-peer discussions prior to surgery. The new requirements are part of United Healthcare's Augmented Notification Process for Spinal Surgeries for patients with UHC insurance. MSOA received an advanced copy of the guidelines and provided several critical comments.
UHC claims the augmented process is an attempt to "improve quality". However, because quality measures are not defined in the policy, it suggests that the real purpose of this model is to diminish surgical options available to the United Healthcare patient. For example, UHC states that the physician has 2 days to conduct a peer-to-peer discussion, but UHC does not specify how available their "peer" will be. Several orthopaedic spine surgeons contacted by MSOA indicated their reviewer is not available after usual patient care hours. This forces the treating physician to choose between conducting discussion or caring for current patients.
